CARCs and RARCs required from January 2027

CMS has announced that issuers must use claim adjustment reason codes (CARCs) and remittance advice remark codes (RARCs) for items and services furnished on or after January 1, 2027.

This is a step toward standardizing remittance advice and explanations of benefits, which vary from payor to payor and pose real eligibility challenges in the arbitration of federal IDR disputes.

The announcement that the new IDR Gateway goes live in late 2026 suggests the new registry and internal open negotiation process will follow soon after.
